Botox vs Kenalog for Treating Keloids: A Modern Approach to Scar Care

Keloids are more than just scars.


They are overgrowths of scar tissue that extend beyond the original wound site. For many, they can be itchy, painful, and affect confidence, especially if located in visible areas like the face, arms and chest.


Traditionally, Kenalog (steroid injections) has been the primary treatment. While it works for some, its limitations and side effects have left patients searching for better alternatives. In the case of keloids, we understand that for many, it is not only about the itchiness and pain but also about the unsightly appearance of the scar and the possibility of recurrence after removal. That’s where Botox (Botulinum toxin) comes in. At Blue Haven Medical, we use Botox as our preferred method for treating keloids because it offers safer, gentler, and longer-lasting results. 


What is a Keloid?

A keloid forms when the body produces too much collagen during the healing process. Instead of flattening over time like a normal scar, the tissue continues to grow, resulting in a thick, raised scar. Common triggers include:

• Surgical incisions

• Acne scars

• Piercings or tattoos

• Cuts or injuries

• Burns


Keloids are more common in people with darker skin tones and often run in families. 


Botox for Keloid Treatment

Most people associate Botox with wrinkle reduction, but its medical uses go far beyond aesthetics. Recent studies have demonstrated that Botox can significantly improve scar reduction outcomes, particularly in keloid cases.


How Botox Works: Reduces skin tension: Botox relaxes the muscles around the scar, lowering the “pull” on the healing tissue and preventing excessive collagen buildup.

Controls inflammation: By calming the local immune response, Botox creates a healthier environment for scar remodelling.

Drawbacks of Botox

The risks and downsides are generally mild, but it is essential to be aware of them.

• Not all patients respond equally to Botox treatment.

• Less effective for very large, mature, or very fibrotic keloids.

• Botox is more costly and typically requires multiple sessions for sustained benefit.


Kenalog for Keloid Treatment

Kenalog (triamcinolone acetonide) is a particulate corticosteroid commonly injected into keloid tissue. It has been the standard of care for decades and is often effective at:

• Flattening raised scars

• Reducing redness and inflammation

• Softening firm keloid tissue


Drawbacks of Kenalog:

Despite its benefits, Kenalog is not without risk:

• Skin lightening or bleaching around the injection site. For patients with darker skin, the risk of uneven skin tone or permanent discolouration can be particularly frustrating.

• Fat tissue loss, which may leave indentations or holes

• Skin thinning, which makes the area more fragile.

• Recurrence is very common, especially without combination therapy. Combination Therapy uses multiple methods to treat the keloids.

In some situations, the best results come from combining both treatments: Botox reduces skin tension and inflammation, while Kenalog targets the bulk of the scar tissue. This dual approach can offer both immediate relief and longer-term control for severe or recurrent keloids, mitigating the high cost.
